According to the NTSB the plane got the wrong fuel.

Just a shame that something so terrible could very easily be prevented with situational awareness and checklists. Just so very sad...

This is exactly why Hoover nozzles and fuel tank filler ports were adopted.

Wally that's because only a certain fuels fuel nozzles will fit into certain fuel ports correct?

The "Hoover nozzles" are the "duckbills" on jet fuel truck sources that won't fit "Hoover rings" on avgas fuel filler ports. Problem is that some Hoover nozzles are themselves fiddly, damaged and sometimes won't fit, so they're swapped for conventional round nozzles. Then there is no backup against the aircraft being serviced with the misidentified fuel type. Problems with the Hoover nozzle fitting are common.
