Risk Assessment For Medical Crew

Hello. I am looking for a good Air Med Crew Risk Assessment Tool (addresses fatigue, sleep, # of flights taken etc etc). I've looked on line and haven't found anything as far as an actual sheet to fill out etc. I am sure there are many computer based models also. I am ok with either but didn't want one that came with a whole 'system' of other things so I think the single sheet paper might be best.

Here at Mercy Air California/Air Methods we have an on-line assessment tool the flight crew members use. It is one page and asks several questions regarding the number of hours of rest received, how many transports completed, etc... This tool totals your score, which is then entered and saved on-line. A supervisor must review this score and approve a crew member to continue working. If you are interested in more details, E-mail me off-line.
