Assessment of orientees

Ladies and Gentlemen:

I would like to poll everyone to see what (or if) your program uses to 
formally assess an orientee's needs for an orientation program.  I am in the process 
of re-organizing the orientation program and would like to develop a tool 
that allows the orientee to assess strenghts and weaknesses.

If you have such a tool, would you be willing to share it, either on or off 
the list?
