Requesting feedback regarding Ativan, Ketamine and Fentanyl Use

Subject: Requesting feedback regarding Ativan, Ketamine and Fentanyl Use

I would appreciate feedback regarding the following questions pertaining to
the use of Ketamine, Ativan and/or Fentanyl.

* How long have you been using any of the above?

* How do you store your Ativan?

* Is your program ground, RW or FW?

* Would a representative from your program be willing to provide a
letter supporting the use of such drugs as Ketamine and Fentanyl?

* For those agencies storing Ativan at room temperature for sixty
days and then replacing it would you be willing to offer a letter support
regarding the storage at room temperature without any complications?

I appreciate any assistance in these matters as we are fighting an up hill
battle with the NYS Bureau of Controlled Substances and NYS Department of
Health.

Please reply to my email address versus the list server. Thank you in
advance!
