trauma criteria

I'm looking for some data regarding trauma criteria utilized by flight crews.  I'm interested in what criteria different flight programs use to determine what level a trauma patient represents and if certain criteria is utilized to deem a flight appropriate.  Examples: >12 inches vehicle intrusion, fall from certain height, etc.  Thank you in advance for your comments and suggestions. Input from Level I trauma centers is also appreciated.
