Dan,
TN. State EMS developed the following QI tool for RSI that you might
find useful. This included the following.
1. Pt wt. in kg
2. Correct meds, dosage and sequence.
3. Initial resp. effort and condition of airway.
4. Method of pre-oxygenation.
5. Tube size, depth of insertion.
6. How tube secured.
7. Difficulty with procedure, # of attempts.
8. If unsuccessful method used to secure airway.
9. Person who performed procedure.
10. Use of C-spine stabilization.
11. Cricoid pressure properly utilized?
12. Ventilation rate and method post intubation.
13. Status of tube at receiving facility.
14. O2 saturation throughout procedure.
While this looks fairly extensive it is all information that should be
documented on every RSI. Its use certainly improved our documentation of
RSIs. Hope this is helpful.

I'm in the process of revising our programs QA for RSI cases. Currently
we are able to tract intubation success, drug administration as per
protocol, age, scene time, ETT confirmation with use of capnography.
Am I leaving anything out?
