Given this scenario, and based on our protocols, I'd decline the
air transport.  Reason being that the dilation is > 4 cm.  Seeing as how this is the patient's first pregnancy, there's not point of reference (Mom doesn't know what will happen).  The whole situation could get too dynamic.  Would offer to perform ground transport of the patient and manage on the ground.
