Geoff,
       Depends on your transport times and the dx. of the pt.  If it is only a 15-30 min. flight and you aren't monitoring ICP's, and the pt. doesn't have a reason to always leave the EVD drain open (i.e. hydrocephalus), I would say to clamp all ports and lie on top of pt.  If you do have longer transport times, I would recommend keeping it leveled and open (which as you know can be a challenge in our environment).  Hope this helps!
