Ventricular Assist Device Transport

Hi All,
I am seeking information on VAD transportation.  I am at the begining of a
huge information gathering process.  For now, I am looking for (from teams
currently transporting VAD's):  inclusion/exclusion criteria, protocols,
modes of transport, crew configurations, education/training, billing
information and anything else you feel is important/significant.
