etomidate max dose




Anyone know of a max dose on adults??  if you use .3mg/kg you can easily go over 20mg per vial --which is all we carry. any thoughts.

Two thoughts. One you can get the etomidate 20cc vials that would give you a total of 40 mg per vial. We carry a couple of these in our drug pack.Two- I can find numerous references in numerous Anesthesia texts of 0.3mg/kg up to 0.6mg/kg for an induction dose of etomidate. As far as a max I don't know if you refer to the initial induction or total dose you can use. There are references of Anesthesia running etomidate 10ug/kg/min IV with N2O and an opiate for maintenance of general anesthesia. The only caution I can find is it states only for short periods of time due to inhibition corticosteroid synthesis.  The Book I refer to is ANESTHESIA by Ronald Miller 5th edition ISBN0-443-07988-9 (set) pages 245-249 Chapter 9 references 237, 246, 248 249.
