	By calculating the total points received per patient, the
following guidelines were established in determining the patient's
chances of survival.


Chances of Survival
0-5 points......30%
5-12 points......60%
Greater than 12 points..95%

Etoh level
	100-299....................2 points
	300-more...................4 points

Injured while riding motorcycle
	With helmet..................1 point	
	Without helmet.................2 points

Arrives wearing biker t-shirt..............1 point

Patient coming from or going to jail...........4 points
Spouse in jail....................1 point

Arrives via air ambulance between 1 am-5 am........1 point

Known by at least five persons in the ED.........1 point

Tattoo.......................1 point each 
(Maximum 5 points)
	If homemade.................1 extra point

Last bath more than two weeks ago............1 point

Forty pounds or more overweight............1 point

"Bad Back" and must be lifted by staff for all x-rays.....1 point

Beard longer than 2 inches...............1 point

Known seizure disorder secondary to previous trauma,
but doesn't know name of medications 
that he/she is taking...............2 points

Routinely "vomicks" two or more times a week.......1 point

Has two children with the same first name.........1 point
Number of living children - n..............3 points
Number of abortions.................1 point each 
(maximum 3 points)
Number of episodes of PID of VD............1 point each 
(maximum 3 points)

Previously shot or stabbed................2 points 
each episode
 (maximum 6 points)

Previous suicide attempts................1 point

Allergic to Tylenol, Aspirin, Codeine, and Darvon......1 point

Allergic to Kwell...................1 point

Misses 3 or more clinic appointments in one calendar year...1 point

Drinks more than one case of beer per week........1 point

Smokes more than two carton of cigarettes per week.....1 point

Bowls in more than two leagues per week.........1 point

Education less than 10th grade..............1 point

Sells blood every month................1 point

On disability.....................1 point

Seeking disability...................1 point

Uses telephone in room to page M.D...........1 point

Arrives in ED with ambient temperature less than
90 degrees F. with no shoes or socks.....................1 point

Arrives in ED with secretions in shoes or socks.......1 point

Arrives in ED with holes or excretions in underwear.....1 point

Number of episodes of frostbite.............1 point

Spits or urinates on ED staff..............1 point
