It is EXTREMELY difficult to legally pay flight staff (or anyone else who does direct patient care) on a salary basis rather than an hourly basis.

Do a search for "FLSA exempt status" and look at the rules yourself.  I would be surprised if an employer who was really cognizant of the rules would allow you to be paid other than hourly.
