I agree. I will note, "strange odor to breath" and back it
up with a fingerstick BS to rule out diabetes. Then, I will document other signs
and symptoms such as those you noted. This is the best method on covering your
butt but not passing judgment on the patient or setting yourself up to be called
into court.
