We just finished a new policy on this issue.  Our policy is that:
   We do transport, restrained prn with restraints appropriate for the patient's medical condition AND which the crew can immediately extricate the patient if needed (e.g., no cuffs; flex cuffs or commercial restraints can be used since we can cut them with scissors).
   Officer is encouraged to accompany.
   Officer must surrender sprays and batons to pilot; also must surrender bulletts.  Pilot secures these items out of reach of all parties.
   Med crew must be prepared to immediately incapacitate patient if needed (e.g., paralytics ready to go).

We've done a couple...have worked fine.
