Re: Charting



I'm a flight nurse awaiting the results for the July California Bar. What I've told our crew is:

Contemporaneous charting (do your writing as soon as possible while it is still fresh). A difficult thing to do some times in our program where we also work as staff in the ED when we are not out. Or back to back flights.

Standard of care: know it, be it, write it.

Habit: try and do things the same way. If you are ever challenged on a procedure, and your memory is fuzzy, "...that's the way I always do it" becomes a credible answer. And, in the alternative, if it is out the ordinary, it's more likely that you will remember it (...because it was not the way I usually do it..."
