FW DNR

I am looking for information on DNRs for Fixed Wing patients.  Being in based in TX, we have the "Texas Out of Hospital DNR" form, but often we are operating out of the state.  We also have a generic form, but it the wording is so complicated, I have to double read it every time we use it. 

Does anyone have a good policy/form they could share that seems to work well throughout the US?
