Advanced skills training

Dear  Flightweb friends:
    I was wondering, for those services not associated with a hospital,
how do you arrange clinical training your nurses for advanced procedures
(cricothyrotomy, needle decompression, intubation, etc..) ? I would also
be interested in knowing how you get ICU experience for your paramedics.
