At my program we currently do two types of follow-up. One is a card with information on the occurance but no patient name, that gives follow-up to the Prehospital folks. The card is sent 7 days after transport. The other is a letter we send to the referring MD with all the information we find out about the patient after they have been in the hospital for ~24 hours. This does include name. Each staff member makes up their own letter since their is not an outline to follow, but most letters are about the same. Only about 1/2 of the patients we fly come back to our hospital, so often times it makes it difficult to obtain information about the patient from other hospitals if the nurse won't give any information (as they are all instructed not to give any information).
