I work at Cincinnati Childrens Hospital.  We have a parent questionnaire
that we give to every patient that we transfer.  Our hospital uses Press
Ganey surveys however, it does not include  feedback about the Transport
Team.   So we decided to send out  our own survey.   Our return rate is
pretty low. (About 50 in 6 months)    Hope that it can help in some
way.
