Advanced Airway Protocol

My service is considering overhauling it's airway protocols, and I have been asked to help develop the new protocol and training program.
If anyone on the list would be willing to share a draft of their programs' entire airway protocol, I would greatly appreciate it. I am specifically looking for how difficult airways and/or failed intubations are handled per your protocols. For instance, when do you resort to an alternative airway (LMA, combitube, retrograde ETI or surgical airway, etc.), and how much leeway does the medic or nurse have in selecting the method and when to use it?
I am also interested in what these programs' initial and continuing training is like where airway interventions are concerned.
I am most interested in rotor-wing flight programs in the northeast that fly with RN/EMT-P crews, I but would be pleased to see any programs' protocol....ground services as well.
       Thank you all very much....

Allan
