We use fentanyl quite a bit with our program as an alternative to MSO4 especially in Trauma patients because it has shorter duration and doesn't cause significant hypotension.

We don't carry them on our flights but you could consider fentanyl or dilaudid as alternatives to MSO4. I am familiar with both from my work in PACU.
