RE: Patient followup procedure for EMS and interfacility agencies
We do a number of evacuations out of Africa to Johannesburg on behalf of
clients abroad.
We have a consent form signed by the patient or a person that would
generally be authorised to do so (spouse, parent etc) that goes into the
patient file together with the in-flight records, authorising the release of
information. If (when) they give us a hard time, and the story regarding
patient confidentiality, we refer them to that form and that seems to solve
the problem.
We also keep a copy to fax to the facility incase they 'lose' it...
With the majority of our clients we're in the position where the patient
remain our responsibility until repatriated to their respective home
countries, so we regard the patient as 'ours' under the care of a medical
facility, as the last resort we'll remind the facility of that...

We are trying to improve our follow up on patient status for agencies we
contact with and are looking for methods or suggestions that have worked for
different flight programs. Some questions that have come up are:
1) Do you keep a log daily and if so who is responsible for keeping it
updated?
2) Any issues with pt confidentiality?
3) How many follow ups do you do for one pt?
4) What is the format of your log?
Any input would be greatly appreciated so we can revamp our system--
