| Beth: Our program keeps a follow-up log that is filled in w/ each transport. We visit and gather general info on all the patients we transport to our hospital for up to 30 days. The EMS agencies we serve know they can call us any time for an update. 90% of all transports return to our facility. Those we transport to another hospital for burns or peds trauma are followed only on an occasional basis through the Trauma Nurse Coordinator. Letters are generated at the time of the call for EMS and upon discharge for interfacility transports. 1) Do you keep a log daily and if so who is responsible for keeping it updated? Yes. Each transport is put in as it occurs. We then visit and gather general info on all patients on Mon/Wed/Fridays. 2) Any issues with pt confidentiality? Not really. When the EMS agencies call for info, they already have the name. We work so closely w/ them that we know most. If there's any doubt, we contact the agency to verify. 3) How many follow ups do you do for one pt? M/W/F for up to 30 days. This has a big impact on our patient satisfaction surveys. 4) What is the format of your log? Name, date of transport, origin of pt/ room#/ Diagnosis, injuries/ Procedures (OR, Cath, etc)/ brief narrative/ and an end block for disposition. Hope this helps.
