In PA, PHRN (Pre-Hospital Registered Nurse) is an additional certification.
RNs who wish to function as pre-hospital providers, and were not previously
paramedics, can take a six month abbreviated Medic program, then sit for
their boards. RNs who were paramedics prior to their licensure, need only
apply to their regional EMS council for a "HP" number (Health Professional
is our official designation). Physicians may also apply for this
certification. In the pre-hospital setting, the PHRN "outranks" the medic,
and is the highest level provider on the truck. Many of us PHRNs disagree
with this, but the state says it is so, so we live with it. Most of us feel
that, in the field we are the same as medics. It comes into play mostly on
interfacility transport where there are modalities not consistent with the
paramedics scope of practice. Many programs, even in the 911 realm, run
dual provider with a PHRN and an EMT-P.
Most of the PHRNs with who I am acquainted work in an ER, either as part of
their program, or per diem.
