 In order to resolve this ongoing "discussion",  I think we need to resolve this with technology.  A giant nationwide network of patient size pneumatic tubes strategically placed at every road intersection and hospital connecting to all trauma centers and specialty hospitals would do the trick.  Law enforcement personnel at the scene could be trained to simply ABC and stabilize the patients with "stuff" in their cruisers then ship the patient to the appropriate hospitals where the person with the most credentials, certificates, licenses, clout, money, or power could eventually take care of the patient.
