Our protocol for airway is 2 attempts by primary intubator  with ETT 
placement.  If unsuccessful then other provider gets one attempt (we fly 
RN/Medic).  If continued no success we try LMA placement.  Then we get to do 
surgical airway.  We have been in service since last September and have only 
had to go to LMA once and that worked.  It is interesting to see what other 
providers guidelines are.
