Excellent reply, Ross.
I would sum it up my feelings (with my limited flight experience) with a
question? Why are we trying to apply a black and white rule to a dynamic and
clearly not black and white problem?  The answer to me seems simple...train
your people to be comfortable in intubating in either situation, then the
choice is made based on the patient combined with the medics/RN's clinical
judgment, not some unrealistic policymakers concept of what field care is.
And most assuredly not because of any fear of a difficult situation. Just my
2 cents worth.
