Many sympathies on this one...what a cluster.  My mind is just racing with the number of things that can and will go wrong.

The program I currently work for uses a BO-105 and there is no way that we would have accepted this mission...especially with the bulging membranes already present.

Other options might include ground transport after delivery in a critical-care unit set up for both mother and infant.  Sounds like they both are going to require some support.

Talking this one over with your Medical Director prior to launching would have been a wonderful asset. 

Overall, this one sounds too risky to move without delivering at the sending facility.
