Scenarios are always fun, especially if you have a few extra minutes to set
them up and a handy EMT student to volunteer as a patient...
I always like to take over a bathroom at the training site and plant my
moulaged patient in the furthest stall (if your volunteer patient will stand
for it), and having him slump in the corner, between the wall and the
commode. I turn all the lights out, and give the team two flashlights to
work with. I have a portable stereo blaring some loud music, C&W or rap
(say it's a bar and you can't get the music turned down) - not loud enough
that the team can't hear each other, but load enough that they have to focus
when speaking or listening. Usually I start out with the patient semi-alert
and have him go unresponsive when they make their first major patient
movement. Sometimes I will stick a small plastic pistol under his waist
band. And of course, have your "normal" PHTLS traumatic injuries that must
be addressed... (students should have all the equipment they need to run
this call, airway bag, backboard, c-collars, etc...)
This scenario demonstrates strengths and weaknesses in communication,
assessing, planning, extrication and treatment.
It's a great first day scenario, especially if you have a group that has
never worked together before. Most of my students are medics, but we do get
an occasional new flight nurse with little or no "street" experience - They
usually comment later on how much they liked the scenario portion of the
class because, while a scene like the above would be a fairly routine call
for street medics, I think many nurses have never experienced anything like
it.
