David - Just a clarification - I didn't mean to imply they are always more 
difficult - but almost every provider thinks that, which is why I recommended 
going with the experts in airway management and watching their techniques and 
perhaps practicing under their eye.  Your suggestions are great - but if they 
haven't been practiced (as many people do get with adults) then it does make 
kids more difficult - the other thing that can make that a peds intubation 
"more difficult" is that sphincter increase you mentioned - if one thinks it 
is (which is usually a matter of comfort level for any skill) then it will 
be.
