Re: peds intubation-- where did I go wrong???
One thing to always keep in mind - little ones can be bag valve masked if the
tube is difficult or several people have tried and you are worried about
airway sweling - just keep the stomach from getting distended ( a feeding
tube as an airvent works fine). I am afraid that sometimes we forget the
basics will work and advanced techniques aren't always in the best interest
of the child. I did neonatal transports - the smallest of the small airways
and frequently several intubation attempts had caused a "bit" of trauma or
swelling so we had to bag these kids for quite a while - monitor the Ox sats
and end tidal CO2 while decompressing their stomachs and they do very well.
Peds can often be difficult and my best advice for getting better is going to
OR with an anesthesia person that does kids alot to watch their technique and
practice if they will allow. Good luck
