You might want to contact Mike Copass at Seattle's Medic One program. They 
have been using RSI for probably greater than 25 years. They might have some 
good data. Ron Stewart had done a longitudinal study looking at training 
methods and success rates of EMS personnel in the early 80's. Although not 
directly relevant to RSI success rates, it was rather interesting in 
comparing the various success rates of personnel trained with manikin, 
manikin & cadaver, and manikin/cadaver/OR rotations. 

That study was done in the last century (I love saying that...back in the 
1980's) in Pittsburgh.
