As was pointed out by others in earlier postings, the problem prompting the black box warning is the development of torsades and/or death in some unknown number of patients that received inapsine. I am not aware of what the doses were in these cases.
Those patients at greater risk incude alcoholics and others with potential deficiencies of magnesium and potassium, along with elderly patients and patients with a history of a long QT intervals - there are probably others at risk that I am forgetting.
By the way, Inapsine's sister drug, Haldol, has also been reported to do the same.
