Insurancs/Managed Care Contracts

Hello All.  A question for the group.  For you who either negotiate or have input into contracts with managed care providers, does anyone have a flat rate charge for triage and transport arranging regardless of the mode of transport (Air vs. Ground).  In other words, does your service set a flat rate fee for contracted managed care providers, and then triage the patient to determine the most appropriate mode of transport (i.e. air vs. ground).  Lastly, without providing any specific numbers, if you do set a flat rate fee, how do you compute and determine your flat rate fee.  I hope I made sense to everyone.  Feel free to e-mail me directly if you wish.  Thanks to all.
