Re: DNR in the Field
Unfortunately, in Kentucky, we face this problem frequently. It is very
frustrating.
In our great Commonwealth, DNR requests can only be on an official "EMS DNR
form". No other document will suffice. It has taken us years to educate the
lawyers, nursing homes, social workers, etc. about existence of this form.
There is no physician involvement.
This form must be filled out in full, and notarized or signed by two
witnesses not related to the patient. It cannot be completed in the heat of
the moment. There is also an option for a hospital style bracelet with an
insert. If the DNR is to be respected, it must be presented to the EMS
personnel PRIOR to starting any resuscitative efforts. The form, or the
bracelet, must be attached to documentation for the run if it is honored.
It can be revoked at any time by either family on the scene, or the patient
themselves. There is no conditional revocation. It is either full
resuscitative effort, or none.
