Re: DNR in the Field



In Ohio this document when signed is the law. If it's signed all the I's
are dotted and all the T's are crossed then it is mandated that we
either stop or we will not start. No matter what is said in the presents
of the family.  This is not an option it is the law in Ohio. Thus far to
my knowledge there has been no legal action taken for not providing care
when a legal DNR is properly filled out.

What we have been running into most often is a blank DNR with the
families wishes to stop. If in doubt we will work the arrest and make an
attempt to contact the attending physician. If reasonable attempts fail
then if asystole is present for the entire arrest sequence then we may
pronounce in the field. This can be either 20 min or 2 rounds of ACLS.
All facets of the arrest have to be in place. IV access, airway, drugs
etc. If any change in rhythm during the arrest sequence we transport to
the nearest ED.
