Documenting Pain scales
New to the documentation requirements by JCAHO is the documentation of a
pain scale, can others decide their practices in this endeavor, PLEASE take
a moment and answer what you can of the following questions for our survey,
we are trying to revamp our process. Sorry to those receiving duplicates
from multiple lists!

1. Do you document a pain scale on your patients? All age groups?
2. What population (neonatal, pediatric, adult, or all) do you
transport?
3. If you do document a pain scale, how often do you document? What is
the minimal amount of time permitted to lapse between documentation's?
4. Do you document a pain scale each time you do a set of vital signs?

5. How often do you do vital signs while caring for your patient?
6. Do you have a quick and easy reference tool you use when you
document the pain scale instead of a lengthy documentation process.
7. At what pain score do you write something in your narrative?
8. Do you document a pain score before, during and after a procedure?
9. What procedures do you not document a pain score?
