transportation waiver

We are in the process of making our patient transportation waiver more
comprehensive. (that we get the patient or relative to sign before we
transport- from one hospital to another- within the country or another
part of the world)

If anyone has any suggestions I would appreciate it.

If you have one you can scan and email, that would be fabulous.
Is there a site where I could get some idea's on this?
