My name is Betty Kovach, RN,BSN and I am the Chief Flight Nurse for Cleveland Metro Life Flight, 2500 MetroHealth Dr., Cleveland, OH 44109, 1-800-233-5433 emergency number, and 1-216-778-3090 informational number.  We do neonates, pediatric and adult transoports.  we also do IABP and VAD transports.  We offer RW, FW and ground critical care.  Ground critical care is mostlly in our northeastern region. I am currently trying to develop a database/contact list for all critical care transport teams in the state of Ohio.  The information I am looking for includes:

Name of program
Hospital Affiliation
Address (at least the city)
Phone #
Contact person
Type of patients transported (Neo, Pediatric, or Adults)
Team composition (RN, MD, RT, EMT-P)
Approximate # of transports per year
Modes of transport (RW, FW, Ground)

If any of you happen to work for one of these programs, or know of any teams located in Ohio, your input to this project would be gratly appreciated.  Please respond directly to my e-mail.
