The cheapest and easiest solution is to use IV tubing.  Cut off a piece 
between the ports, thread both ends thru the roller clamp (difficult but it 
can be done). Throw a clove hitch (ask any Boy Scout how to do this) around 
the ETT.  Take the loop around the patient's head and pull the roller clamp 
up tight to secure.
